This complete form is required for your child(ren) to participate in West End Baptist Church events and programs from September 1, 2025 - August 31, 2026. (*Required fields) 

Parent Permissions

Parent/Guardian named below gives permission for student(s)/children to participate in events and programs at West End Baptist Church. Completing the permission form will allow student/children photos to be taken and used for church publications or social platforms. 

Permission is given to WEBC to transport child(ren) to church events. 

Parent/Guardian releases WEBC from all liability, claims or demands for personal injury, including sickness. In case of an emergency injury or illness, Parent/Guardian gives permission to seek treatment for child(ren). Parent/Guardian acknowledges that they are responsible for all expenses related to any emergency medical care.
